Dear Student, 

 

An integral part of your internship experience is the demonstration of your ability to assess your performance. You are required to complete and submit this self-evaluation to the Office of Field Education at least two weeks prior to the completion of your hours. The estimated completion time for this evaluation is approximately 30 minutes.

 

Set Aside: Approximately 30 minutes of uninterrupted time. The estimated completion time for this self-evaluation varies depending on the level of prior preparation.   

To complete this evaluation you will need:

1.   Your field instructor's name and valid e-mail address

2.   Your ELA

 

Once you have assembled the items above, you are ready to begin the evaluation.

Once you have completed this evaluation, your comments will be sent to your Field Instructor along with a link to the Field Instructor evaluation.  You will have an additional opportunity to provide feedback on your site through the Student Assessment of the Practicum Experience (SAPE).  Please schedule a final meeting with your Field Instructor to discuss your Self Evaluation as well as the Field Instructor Evaluation.
