Assessment Dispute Form

Students wishing to dispute a specific assessment, not a competency decision, must complete this form within 14 days of scores/forms being released in Keystone.  Disputes of exam, standardized patient/OSCE, or patient safety simulation can only occur if a proctored review is completed.  Students wishing to conduct a proctored review must contact OMSE to schedule a time to review their assessment. While students may choose to review their assessments at future reviews, if these reviews occur after the 14 day window, disputes will no longer be allowed. This form will be sent to the Assistant Dean for Assessment and OMSE in order to share with the appropriate parties.

The appropriate parties with whom dispute forms will be shared are as follows: Observed Structured Clinical Examination disputes are directed to the Clinical Skills and Communication Thread Leaders.  Exam related disputes are directed to the specific course/module leadership. Writing assignment disputes are directed to the specific thread leader. JITs and global assessments disputes are directed to the appropriate course (Immersions or clerkship) leadership.  End of Clerkship Narrative disputes are directed to the appropriate Clerkship Director. Disputes for all other individual phase assessments are directed to the Chair of the CAC who will reach out to other responsible parties as needed.

Please note, students may work with the Immersion or Clerkship Director to clarify or seek further understanding of assessment outcomes from frontline assessors and should not discuss corrections or desired changes to assessment outcomes directly with an assessor.
