Dear Student,
We value feedback on your practicum/internship experience! As a student who has completed a practicum/internship at your agency you have insight and perspective that is unique and valuable. The Student Assessment of Practicum Experience (SAPE) is an opportunity to share vital information about quality and learning in the practicum site.
The SAPE is divided into three sections. The first section collects information that will be shared with other students interested in learning more about the student experience at your practicum/internship site. The second section collects information about the practicum/internship site that is not shared with other students. The third section collects information about your experience with field faculty advisors, this information is also not shared with other students.
Please complete this survey at the
conclusion of your practicum. If you are completing a two-semester practicum, please complete the evaluation at the end of your entire experience.
The SAPE is confidential and is
designed to:
1. Inform the Office of Field
Education (OFE) about the quality of the
student’s learning experience.
2. Provide information about the practicum site for evaluation and training.
3. As appropriate, be reviewed by
students searching for future practicum/internship placement.
Please note: Whenever possible, constructive feedback will only be shared with field instructors in an aggregate manner and on a delayed time frame in order to preserve student confidentiality.
If warranted, a professional from OFE may contact you
regarding this evaluation for additional information about your responses.
