Dear Student,
An integral part of your practicum experience is the demonstration of your ability to assess your performance. You are required to complete and submit this self-evaluation at least two weeks prior to the completion of your entire practicum hours. A copy of this self-evaluation will be emailed to your field instructor upon completion.
Preparing to Complete the Self-Evaluation
Set Aside: Approximately 30 minutes of uninterrupted time. The estimated completion time for this self-evaluation varies depending on the level of prior preparation.
Assemble Required Information:
Field instructor's name and e-mail address
WU Student ID number
Copy of your MPH and MSW ELAs
